First Time Clients & Patients

If you are a first-time client, please let us know where your pet has been receiving medical care, so we can request their history/records before your appointment. If you prefer, you may bring the records with you when you come for the appointment, and we can scan them into their new medical record. If you don’t have access to a medical history/record, previous receipts are an acceptable alternative.

We recommend all pet owners keep a record/journal of their pet's health and environmental changes (moved, adopted new pet, family changes, etc.). It is especially helpful to have dated records of changes in behavior. If you have something like this, please bring it to your appointment or send us a copy by email. If not, let us know if you suspect any correlations. The more information you can provide, the better we can assist you and your pet with any medical or behavioral issues that arise.

Whole Health Veterinary Care is not a full-service emergency facility. We do see critical care cases as the schedule allows. However, if your pet needs emergency medical care, please contact or proceed to an emergency facility.
